Ending Veteran Homelessness in New Hampshire

Ensuring community-based systems are in place to quickly house a Veteran should they experience homelessness or be at risk of experiencing homelessness and transition to permanent housing stability. The Ending Veteran Homelessness in NH Project is to create a system that engages Veterans within 24 hours of them being entered into Coordinated Entry (CE) allowing Veterans quick efficient and simple of access to permanent housing within 90 days.

The project will result in a sustainable system in which providers who encounters a Veteran experiencing homelessness or at risk of experiencing homelessness knows their role and how to connect a Veteran to that system. The output of the project will include sustainable data for submission to USICH consistent with a state that has met USICH metrics for Ending Veteran Homelessness.
